Non-Controlled Drug and Supply Request Form
Description
Request forms for non-controlled drug and supply for work to be completed under an approved animal use protocol.
All supply order requests will be charged a $15 fee per order form submission starting 7/1/2026.
All orders will be placed twice a month (on or about the 15th and 30th). Items may take up to two weeks for delivery. The person who requested the order will be contacted to schedule a pick up from the ACP main office.
